Spousal transfers: the unlimited one

Any asset, any amount, any time. Disabled-child transfers: broad but proof-heavy

Transfers to a child who is blind or permanently and totally disabled carry no penalty regardless of the child's age or the asset's size. Disability is proved by the federal standard, most cleanly by an existing SSI or SSDI determination. Transfers into a properly drafted trust for that child's sole benefit qualify the same way.

The under-21 home transfer

The home, specifically, can pass to your child under 21 without penalty. It is narrow and rarely used, but when it applies it removes the home from both the eligibility count and later estate recovery.

The caregiver-child exception: the crisis workhorse

The requirements are precise. The adult child lived in your home for at least the 2 years immediately before you entered care, and the care they provided is what kept you out of a facility during that time. Ohio wants both facts documented: the notarized ODM 10271 caregiver affidavit describing the services, their frequency, and their duration, and a physician's certification that the care delayed institutionalization.
